2026 Application | The Surrogacy Foundation

Thank you for your interest in The Surrogacy Foundation (TSF) grant. This application is very comprehensive and will require you to provide in-depth information about your financial and medical history, as well as forms that will need to be downloaded and filled out by a physician. TSF needs all of this information to make the best possible decision, and TSF will treat it with the utmost confidence. Only those who are on the review committee will have access to this information.

The deadline to submit your application is September 30, 2026. TSF will try to make a decision by December 15, 2026.

Note on Confidentiality:

The Surrogacy Foundation acknowledges that some of the information and forms required to complete this application are sensitive medical and financial data. TSF takes your privacy very seriously. Any sensitive data will be disclosed solely on a need-to-know basis, and everyone who has access will be required to sign a confidentiality agreement prior to review. Additionally, the platform used to administer this application is compliant with relevant regulations. Please feel free to reach out if you have any additional questions.

We understand that parts of this application may ask about sensitive medical, financial, emotional, or family-building experiences. Please share only what you are comfortable sharing and what you believe is relevant to your application. You do not need to provide graphic details. Our goal is to understand your eligibility, readiness for a surrogacy journey, and how this grant may help you move forward.

Eligibility Requirements

Please review these requirements carefully. Applicants must meet ALL requirements to be considered for The Surrogacy Foundation grant.

Is the surrogacy medically necessary?

Surrogacy must be medically necessary, including but not limited to:

-Absence of uterus (congenital or acquired)

-Significant uterine anomaly (e.g., irreparable Asherman's syndrome; unicornuate uterus associate with recurrent pregnancy loss).

-Absolute psychological or medical contraindication to pregnancy (e.g., pulmonary hypertension)

-Serious psychological or medical condition that could be exacerbated by pregnancy or cause significant risk to the mother or fetus.

-Biological inability to conceive or bear a child such as single male or homosexual male couple

Are you a citizen or legal, permanent resident of the United States (U.S.)?

Do you have a minimum of three euploid (genetically normal) embryos?

I understand that the embryos must be created (and frozen) by the time of application deadline.

Do the frozen embryos reside in the United States?

I understand that if selected as a winner, I agree to use Surrogacy Professionals only located in the United States.

I understand that if I am a finalist, I will be required to have a Criminal Background Check.

If I am selected as a winner, I agree to be a Spokesperson for The Surrogacy Foundation.

Have you already funded your surrogacy journey in any amount through escrow?

Please describe the funding source (HELOC, family loan, savings, etc.), amount available, whether the funds are currently available, and what expenses the funds may be used for.

NOTE: Our grant is typically reserved for those who haven't funded escrow yet, however, we may consider if you have only made your first deposit. We DO NOT reimburse regardless of the answer.

Personal Statement:

Help us get to know you. Personal statements should be no more than two pages, and photos are allowed.

Please share as much of the following as possible and to the extent you are comfortable:

Tell us about yourself and your family-building journey.
Summarize your medical need for surrogacy.
Describe how you have learned about or prepared for the surrogacy process.
Describe your parenting aspirations.
Describe your personal or professional support system.
Describe any preferences or expectations you have for your surrogacy journey.
Share anything else you believe reviewers should understand.

Medical: Please have your Reproductive Endocrinologist (REI) complete and sign the medical form. Please allow sufficient time for your physician to complete the form and submit to TSF by the application deadline.

Financial: Please attach all of the following:

• The Surrogacy Foundation (TSF) Applicant Financials

• Last two (2) Federal income tax returns for each applicant

• Two (2) most recent pay stubs for each applicant

• Two (2) most recent Forms W-2 for each applicant

TSF National Grant

Our flagship grant provides $100,000 deposited directly into an escrow account that goes toward base surrogacy journey expenses such as embryo transfer, agency fees, legal fees, surrogate base compensation and other allowances. Families may work with any professionals of their choice provided they are located in the US. We will also offer a list of professionals who are willing to offer their services pro bono to the grant recipient. Variable expenses are not covered, which include but are not limited to expenses related to supplementary insurance, lost wages for extended required bed rest, or fees for twins and c-section.
